- Appointments are preferred for all services.
- The Pratt County Health Department can bill all insurance companies including Medicaid for services rendered. Patient is responsible for any remaining balance not covered by their insurance.
- If you do not have insurance, payment arrangements can be made. Most services will not be denied for patients' inability to pay.
- What to bring with you to your appointment:
- Photo ID
- Insurance/Medicaid Card (if you have one)
- Adoption/Foster Parents - Must bring court custody papers/placement papers.

- Drug Screen Collection Kit Must Include:
- Specimen Container
- Chain of Custody Form
- Fed Ex Mailer Bag
- Shipping Label
- Family Planning Appointment:
- Any paperwork that was mailed to you prior to your appointment.
- For Sliding Scale Services, you must bring proof of income from all members in the household.
Accepted proofs of income:
- W-2 form
- Most current pay-stub
- Last year's tax statement
- Disability award letter
- Social security award letter
- Unemployment
- Child support
- Immunization Appointment:
- Historical Vaccine Records. Please send us any records of vaccinations from a different facility prior to your appointment via fax, email or drop off.
- WIC Appointment:
- Proof of ID for Caregiver and Child
- Proof of Income
- Proof of Residency
